Epipactis leptochila has large leaves that are longer than the distance between nodes with, generally, a bright spot at the base. The spike bears numerous and largely opened flowers. The epichile is characteristic, it is particularly narrow and heart-shaped.

Two varieties can be observed, the type-species with a straight forward epichile and the variety neglecta GEVAUDAN 2002 which differs by its epichile clearly directed backward with the tip on the side.

Flowering season from July to August.
